Puskesmas Cikaum Subang represent the peripatetic society Health Center in the field of service of Society health.As for purposes and objectives from this research is to learn the process of healt service which still use the manual system into compurized system in Puskesmas Cikaum Subang. System scheme used by method of Prototype, used appliance to design system in the form of Flowmap(scheme emit a stream of document) contex diagram ( contex Diagram)DFD (Data Flow Diagram), and technique of data collecting by using research method by Interview and Survei. While appliance of development of data base application use Paradox 7.0. This information system will gain the work effektiviness and time efficiently in processing the “ Patient Data” producing puskesmas services and human resources optimalizing and helps giving information for it’s own, specially for the patient whos making an medical appointment in the puskesmas in generally. This system also have the weakness from facet of program facility which in rough is so that needed by some repair.
