Asshobirin Islam Hospital is an Office Government which was rounding in Health Serves. Every instantion want that health service is the best. The lack of care house special purpose application or program in Asshobirin Islam Hospital is one of the reason why the loss of data in a document format and assignment report, service report, administration report are usually late. So therefore, the writer propose the research sub division of care house installation to design health service information system. Moving on that purpose, we did some method, consist of collecting data, observation, system analyst, used prototype development system and UML object orientation with use case, actor, class diagram, sequence diagram, collaboration diagram. It was using Application Program and included database. Service progress at Asshobirin Islam Hospital are including department of registration, health service and billing, so the new system should has integrated the progress and transaction, with the Information System of Health Service, will be support and help for health service in shortly time, accurate and in integration the others one.
