Puskesmas represent tip of lance in realize Departmental mission and vision Health which among others to improve performance of Puskesmas, by improving to access society to service of health which with quality, improving surveilans, health information system and monitoring .Puskesmas in Town of Cimahi, process - the process conducted manually it is of course will eat loads of time and also will face mistake risk which easily earn happened manual processing so that in the end will affect at performance of Puskesmas pertinent. Process model to be used in Information System Service of Puskesmas. pursuant to paradigm of Waterfall ( model cycle life waterfall standart) what consist of Requiretments, Design, Coding, Testing, and Operation. As for appliance assist in scheme of system to be used with Schema Emit A Stream Of Document ( Flowmap), Diagram Context, Diagram Emit A Stream Of Data ( Data of Flow Diagram), Dictionary Data ( Data of Dictionary), Specification Of Process, ERD ( Entity Relational Diagram) and normalization. With existence of this Information System can assist to lessen depended from many stuffing form at stuffing form and also summarize data - operational data computerized so that more effective and is efficient, data storage media one place and more well guaranted its strength, efficient so that in the case of prepared room.
