information system of Levying goods and service is the information system which it’s purpose in easier helping in the case of levying goods and service. The information system is needed for more increase the effective and efficient job all involved department. levying goods and service it self conducted if there is material requisition and service with aim to fulfill goods decrease or requirement or service for the activity of produce or activity of other support. processing of existing document still have the character of manual, for example in the case of record-keeping of transaction evidence, archives and also making of report, so that will decrease job effective and efficient, because of by using processing of manual document, forwarding of required information will be tardy. The steps to be done include from step of system analysis with system approach method that oriented at the object is visualized by UML (Unified Modeling Language), where as to design and implement for it is used Microsoft Visual Basic 6.0 programming language. the system developing method be used is prototype method. By the information system of Levying goods and service at PT Pupuk Kujang Cikampek, transaction evidence archives and record-keeping no longger use ordinary document, compilation of report no longger use manual system but is computerized, so that the information is needed can be presented as quick, appropriate and accurately. ii
